Helping patients home after discharge

A special event to raise awareness of the many community and voluntary organisations that offer valuable services to patients following discharge from hospital has been held at the Royal Berkshire Hospital.

 

The afternoon provided an opportunity for providers to showcase the support they can offer and for staff to increase their familiarity with these services.

 

Trust occupational therapist and organiser Amy O’Neill said: “We had a really good attendance from therapists and nursing staff from across the hospital. There are so many organisations in the community and voluntary sector that offer support and we know we must work all together to use every resource available to help ensure patients are discharged. This was a good chance for our staff to refresh their knowledge on the many services which can assist patients to return home promptly, safely and well supported. Thank you to everyone who came along and took part in this valuable event.” 

 

Mary Sherry, Chief Operating Officer said: “It was great to see so many Trust and community and voluntary services presenting together on how we are working to get patients home from hospital safely and in a timely manner. There is clearly a lot ‘out there’ to help this happen and the partnership working is a core part of our approach. These services make a real difference to patients’ lives.  A big well done to everyone involved.
