Postnatal care - Community

All mothers and babies who are discharged from hospital, MLU or home birth will receive a face to face appointment on the day after being discharged from hospital or given birth at home.

If during a video consultation the midwife decides a face to face appointment is required, this will be actioned without delay.

  • Video consultation with the community team
  • Visit to be arranged if required

  • Face to face appointment with the community team
  • Newborn bloodspot screening
  • Baby's weight is checked
  • Full postnatal check for mother if midwife appointment required
  • Full postnatal check for baby.

  • Video consultation with the community team
  • Visit to be arranged if required

  • Face to face appointment with the community team
  • Discharge from maternity services (around days 10 - 14)
  • Baby's weight is checked (if required)
  • Full postnatal check for mother and baby.

We will usually transfer your care to the health visitors on, or around, day 10 - 14.

Do make a double appointment with your GP for both yourself and your baby to have a face to face check-up at around six to eight weeks after the birth.
