If this is your first visit, the
receptionist 
at your clinic will need to record your
personal details, including your postcode, your NHS number (on your
medical card) and your GP's name and address.
If you are taking any medicines or
tablets, please bring them with you. Your appointment letter will
tell you if you need to bring a urine sample.
If you are unable to keep your appointment, please ring the
department concerned and let them know - another patient may be
glad to take your slot.
Not all clinics or departments have their
own outpatient area. In this case, ring the patient services
team on 0845 900 7000 if you need to change or cancel
your appointment.
