This form is provided to enable GPs and practice managers to log queries, recommendations, complaints or compliments with the Royal Berkshire NHS Foundation Trust. 

Please limit your formal complaints to only the most serious issues relating to patient care or safety. 

We will then assess how best to address your query and will respond within three working days letting you know what action we will take.
