Below can be found all the proformas that
have been designed to automatically pull all the demographic
information from your clinical system when making a 2 Week Wait
referral.
These proformas have been agreed in
conjunction with the RBFT consultants, the Thames Valley Cancer
Network, Berkshire West PCT and GP groups.
Please ensure that all the relevant
questions are completed with as much information as
possible.
We recommend that you read the installation procedure for your
clinical system before installing. Once
read simply click on
your clinical system folder below to display the 2 Week Wait
proformas and click on the proforma you wish to open or save.
Please note we do not currently have any installation
procedures for iSoftsystems.
